Need a Provider

Individuals with developmental disabilities have the right to choose any available, qualified provider statewide. Individuals may select these individuals or agencies to assist them in their daily lives. At the Van Wert County Board of Developmental Disabilities, a person’s SSA (Service and Support Administrator) will assist them in making that selection.

Waiver Service Definitions

Homemaker Personal Care: Care that is provided in a person’s home or in a community setting to assist an individual with daily living skills such personal hygiene, dressing, eating, housekeeping, medical appointments and community outings .

Transportation: A service offered to assist individuals in gaining access to waiver related services or other community amenities, activities or resources identified in the individual’s plan.

Informal Respite: Short term support provided to an individual receiving waiver services. This allows family members personal time away as identified in the individual’s plan.
