How to Become a Provider

Individuals or agencies providing services to persons with developmental disabilities must obtain certification from the Ohio Department of Developmental Disabilities. Because the health and safety of those receiving waiver services is of primary importance, the Department reviews all applications thoroughly. The applicant must provide the necessary information and documentation to establish that all qualifications and standards for certification are met.

Receiving Your Certification – What Happens Next

Once a person is certified to be a Medicaid provider they will be assigned a unique number required to obtain federal reimbursement for services they provide. After a person is certified, they will be able to provide services under the Level One and Individual Options Waivers. Independent providers must provide services directly and are not permitted to employ others to provide services on their behalf.

Applying for Certification – What You Need

You will need the following documents when applying:

• Verification of age -all applicants must be 18 years of age or older
• High School Diploma/GED
• State Identification – Valid driver’s license or other government issued photo identification
• Social Security Card
• First Aid Certification
• CPR Certification

You will also be required obtain a current report from the Bureau of Criminal Identification and Investigation (BCII). Additionally, if you have not resided in Ohio for the past five years, you will need an FBI check.

If you are seeking certification for Transportation Services you will need the following:

• Valid driver’s license
• Recent copy of your driving record from the Ohio Bureau of Motor Vehicles with less than six points on it
• Proof of vehicle insurance
