We utilize Person Centered Planning to help individuals outline their needs, wishes and goals. This pursues social inclusion through means such as community participation, employment and recreation. Person Centered Planning insists that discussion is centered on what the person is telling us is important to them, with their words and behaviors. It helps to engage participants and solve difficult problems.
What age group does the Early Intervention program serve?
The Early Intervention program serves children birth to three years old that have a developmental delay, disability or a medical diagnosis.
How much do your programs cost?
All services are provided at no cost to your family. Your PSP will use a coaching model of service and will suggest strategies to try with your child during the time between visits.
What age do you have to be to qualify for Adult Day Services?
Adult Day Services are available to individuals over the age of 16.
What type of activities are done for the Adult Day Services program?
Adult Day Services provide education opportunities and other important skill development for individuals with developmental disabilities. They are designed to strengthen a person’s quality of life, may be provided in various settings and can include social, leisure and recreational activities. Available services include:
- Personal care
- Self-determination training
- Fitness and wellness training
- Volunteering
- Community inclusion

Transportation is available to those receiving Adult Day Services – at no cost to the individual.
What services do you offer for individuals aged 3 - 22?
Service and Support Administration is a service for children (age 3 and above) and adults with developmental disabilities. The Service and Support Administrators (SSA) assist individuals, their families, and guardians by helping them to be active participants in their communities. Service and Support Administration services are available to Van Wert County residents who have evidence of a developmental disability before age 22.
